Scientific Background: NHE3 (Sodium-hydrogen exchanger 3; Slc9a3) is an epithelial transport protein that carries out 1:1 exchange of Na+ and H+ across the plasma membrane. It is found in the apical brush border membrane of the proximal tubule in the kidney and the small intestine, and on the basolateral side of pancreatic/duodenal cells. (1). It features an N-terminal membrane domain for transport and a C-terminal regulatory domain. It is highly regulated by the C-terminal cytoplasmic domain, which binds to regulatory proteins (e.g., NHERF1/2) and links to the cytoskeleton. NHE3 is phosphorylated and regulated by multiple kinases including PKA, SGK1 and CK2A. It is inhibited by phosphorylation via PKA (activated by PTH/dopamine). It can be phosphorylated by calyculin A, and dephosphorylated by PP1 catalytic subunit in vitro (2). Defects are associated with congenital secretory sodium diarrhea. Reduced function can lead to lower glomerular filtration rates, while its regulation is involved in hypertension and heart failure.
