Scientific Background: The Kir2.1 is a strong inward-rectifier potassium channel protein, encoded by the KCNJ2 gene, and is essential for stabilizing the resting membrane potential and regulating action potential duration in excitable cells, particularly in the heart, skeletal muscle, and brain. It forms a homotetrameric channel consisting of four identical subunits, each with two transmembrane domains (M1, M2), a pore-forming P-loop, and large cytoplasmic N- and C-termini. Channel activity is promoted by phosphatidylinositol 4,5-bisphosphate (PIP2) binding at the C-terminus, while inward rectification is caused by intracellular polyamines and Mg2+ blocking the channel during depolarization. Mutations in KCNJ2 cause Andersen-Tawil syndrome, short QT syndrome, and atrial fibrillation, often resulting in severe cardiac arrhythmias and developmental defects (1).
