Scientific Background: HSPB2 (Heat Shock Protein Family B Member 2; MKBP) is the most divergent member of the sHSP family with only 30% sequence identity to all other mammalian sHSPS (1). It features a conserved alpha-crystallin domain at the C-terminus, which is characteristic of the small heat shock protein superfamily. It is an ATP-independent chaperone that maintains protein stability and protects cells against stress-induced damage, particularly in muscular and nervous tissues. It facilitates neuronal regeneration via autophagy following traumatic brain injury. HSPB2 is known to associate specifically with myotonic dystrophy protein kinase (DMPK) in skeletal muscle. HSPB2 enhances the kinase activity of DMPK and protects it from heat-induced activation. HSPB2 also shows a unique nature compared to other sHSPs, in that the expression of HSPB2 is not induced by heat shock (2). In unstressed skeletal muscle, HSPB2 forms large oligomeric complexes with HSPB3 in the cytosol which are localized on mitochondria and the neuromuscular junction (1, 3). During stress, these complexes dissolve and MKBP’s localization to mitochondria increases, leading to increased cell survival. A distinct role for HSPB2 in terms of cardiac mechanics is that it is required for normal systolic performance and normal cardiac energetic (1). It is expressed in cardiac muscle (left ventricle), skeletal muscle, and tissues like the placenta and various fetal tissues. HSPB2 has also been found to be expressed in several cancer cell lines, including human breast cancer, indicating that HSPB2 may be an important factor in tumour transformation and metastasis (4). In bladder cancer, low expression of HSPB2 is linked to higher risk for relapse. It is also known to inhibit apoptosis in cancer cells.
