Scientific Background: GABRA4 is an alpha subunit of the heteropentameric ligand-gated chloride channel gated by gamma-aminobutyric acid (GABA). The GABA-A receptor is a member of the superfamily of fast acting ligand-gated ion channels. The individual subunits of these receptors have similar sequences and structural features (1). It commonly binds to GABA to regulate brain excitability and is associated with autism, epilepsy, and developmental delays. GABA-A receptors are the major fast inhibitory neurotransmitter gated ion channels in the brain (2). GABAARs containing alpha-4 are predominantly extrasynaptic, and contributie to tonic inhibition in dentate granule cells and thalamic relay neurons. GABAAR containing alpha-4-beta-3-delta subunits can simultaneously bind GABA and histamine where histamine binds at the interface of two neighboring beta subunits, which has been implicated in the regulation of sleep and wakefulness. In addition to its role in the brain, GABRA4 is essential for mediating autophagy and managing proinflammatory responses in macrophages, acting as a regulator of innate immunity. Mutations and variants in the GABRA4 gene are linked to epilepsy, developmental delay, autism spectrum disorder, and behavioral abnormalities.
